Methods
  • This prospective cohort study with a control group involved 93 patients. Between 2008 and 2010, 43 patients underwent the laparoscopic transperitoneal sublay mesh repair.
  • During the same period of time, a control group of 50 patients underwent an open sublay repair after Rives and Stoppa.
  • In 2011, all patients were invited for follow–up.
  • This included pain assessments and physical examinations with use of ultrasound.
Results
  • The two groups were comparable in terms of patient characteristics and hernia data.
  • The operating time was slightly longer for the laparoscopic technique. The hospital stay was shorter in the laparoscopy group.
  • There was less chronic pain in the laparoscopy group, but this difference was not statistically significant.
  • There was no significant difference in postoperative complications, use of analgetics, foreign body sensation, and paresthesia between the two groups.
  • The authors found one long–term hematoma in the laparoscopy group and one seroma in the open group. In this series, there were no recurrences and no wound infections.

If you are contemplating lap band surgery then you might well be encouraged by the results of an Australian study which reported that, in a controlled test, lap band surgery proved to be far more effective in the long term than a very restrictive 500 calorie diet. However, before you rush off to your surgeon, you might just like to take a moment to examine the study closely.
The study involved a total of 80 patients half of whom were given lap band surgery and half of whom were put on a very low calorie diet of just 500 calories.
After six months both groups were showing similar results and had lost about fourteen percent of their starting weight. However, at the end of two years the results for the two groups looked very different.
The lap band surgery patients had lost about twenty two percent of their starting weight and showed a marked improvement in their health and quality of life. By contrast, the dieting group had re-gained much of the weight they lost in the first few months of their diet and, on average, were only just over five percent below their starting weight.
This result would seem to confirm what many people, including most doctors, already know and this is that dieting, other than to lose the odd pound or two now and again, really doesn't work. It would also appear to make a strong case for gastric banding.
Lap band surgery is one of several options open to patients as a solution to the problem of morbid obesity and is generally only considered once a patient's BMI reaches 40. Exceptions can be made and lap banding offered to patients with a BMI as low as 35, but this is normally only done when the patient is also suffering from a potentially life threatening medical condition which is linked to being overweight. For the majority of patients this means that lap band surgery would normally be considered at the point at which they are about 100 pounds overweight.
In the case of this study however the patients chosen for the study were just 50 pounds overweight. So, can we really rely on these results when this is not the target group to which lap band surgery would normally be applied?
The truth of the matter is of course that lap band surgery can be very effective in cases of morbid obesity and is proving to be a very popular choice with many patients. As with all forms of obesity treatment however it is vitally important that we collect sound data so that the various different methods can be evaluated and patients given accurate advice on which they can base their treatment decisions.
Studies of this nature certainly do not add to the base of knowledge which we need to be building and have more to do with grant funding from a lap band manufacturer than they do with any desire to advance the treatment of obesity.
Of course that is only the writer's view and you may well disagree.

First we need to define what we mean by a particularly high BMI. It is generally accepted that obesity starts at a BMI of 30 and becomes morbid obesity at 40. This is the point at which most doctors will consider a surgical solution to the problem. With a BMI in the 40s patients are generally suitable for all forms of weight loss surgery (unless excluded by such things as co-existing medical condition) however, as BMI passes 50, the risks of gastric bypass surgery rise steeply and choices become limited. Against this background we would define a particularly high BMI as being 50 or above.
Despite the increased risks, some patients are nonetheless considered suitable and do opt for gastric bypass surgery while others take the safer route of gastric banding, but which is better?
In a recent study involving 106 patients (all of whom had a BMI in excess of 50) 60 patients underwent gastric banding while the remaining 46 opted for gastric bypass surgery. More than three quarters of the gastric banding patients suffered complications, which included dehydration and vomiting, while less than one third of the gastric bypass patients suffered complications. In addition, the gastric banding patients experienced only about one third of the weight loss recorded for the gastric bypass patients.
Perhaps most surprisingly, more than three quarters of the gastric bypass patients were satisfied with their result and none regretted their choice. By contrast, less than half of the gastric banding patients were happy with the results and ten percent actually expressed dissatisfaction or regretted their decision.
On the surface therefore it would appear that gastric bypass surgery is clearly the preferred choice. However, for many patients with a high BMI this is simply not an option as the risks are simply too high, so what is the answer in these cases?
Without doubt the preferred option is a combination of restrictive surgery, such as gastric banding, in the first instance to reduce weight to less than a BMI of 50 and then to follow this up with a second form of absorption surgery, such as the gastric bypass. In many cases however, rather than gastric banding, the preferred choice is to use a vertical gastrectomy (sometimes referred to as a vertical sleeve gastrectomy) as the first element in this two-stage surgical process.

Any type of surgery comes with a certain degree of risk, and weight loss surgery (also known as bariatric surgery) is no different. But what are the potential complications of weight loss surgery, and how common are they? We will attempt to answer these questions here.
What is Weight Loss Surgery?
Before we talk about the possible complications of weight loss surgery, we need to define what this type of surgery entails, just so we are on the same page. Simply stated, bariatric surgery and weight loss surgery both refer to the same thing -- various types of surgery intended to help obese patients lose weight.
In her book The Patient's Guide to Weight Loss Surgery, author April Hochstrasser defines bariatric surgery as "surgery on the stomach and/or intestines to help people lose weight by altering the normal digestive process."
Complications of Weight Loss Surgery
The complications associated with weight loss surgery vary, based on the type of surgery being performed. And, of course, the potential risks and complications of general surgery apply here as well.
Potential complications of general surgery:
* Respiratory problems
* Risk of infection (from incision area)
* Excessive bleeding (internal or external)
* Allergic reaction to medicines
* Reactions to anesthesia
Potential complications of weight loss surgery:
* Leaking of stomach acid or bacteria into the abdominal cavity
* Vitamin and mineral deficiencies as a result of malabsorption
* Hernia in the abdominal wall (with gastric bypass)
* Dumping syndrome (heart palpitations, nausea, sweating, etc.)
The major risks of weight loss surgery:
* Leaking of stomach content into other areas
* Blood clots
* Respiratory failure
According to the University of Iowa report "Mortality in Obesity Surgery," the majority of patients who die within a month of weight loss surgery suffer one of the three major complications above.

LASIK eye surgery can completely change the way you see the world around you. It can take your blurry vision and make it clear. It is reasonable to have high expectations, but you should be clear on what the results may be.
Part of getting the most from LASIK surgery is also choosing the right doctor. You need to ask a lot of questions about your surgeons experience and the overall LASIK experience. Your surgeon should be experienced with using the equipment and with doing LASIK surgery. He should be able to answer any question you have as well.
To get the most form LASIK surgery involves you being prepared for the surgery. It is not all just about getting perfect vision. You want the whole experience to be a good one. The most you can expect is to walk away being grateful and happy that you had the surgery it is then that you have really gotten the most form your LASIK surgery.

While Lasik eye surgery has been marketed as a quick and painless way to improve the vision of those afflicted with nearsightedness, Lasik eye surgery may do more harm than good if performed negligently.
Lasik eye surgery procedures are meant to correct faulty or myopic vision, sometimes this procedure has tragic results. To lose your vision as a result of the negligence of a medical professional is a very serious thing.

Undoubtedly, thousands of Americans see better because of Lasik eye surgery - but this procedure is not necessarily right for everyone. Cosmetic plastic surgery is more popular than ever for many different reasons. But even though this type of surgery can alter and improve any area of the body, there are risks and issues to be aware of. Here is a quick look at some of the more serious issues with this procedure.
One of the bigger risks or side effects of cosmetic plastic surgery will always be infections. Infections can occur from a couple of days following surgery until 2-3 later. It is critical to be aware of this as it can lead to serious issues.

The high prevalence of all symptoms in the community, regardless of their
nature, was first described in the Peckham experiment in 1946 [31] and subsequently
by Wadsworth [32] and Hannay [33].These studies demonstrated
that most people have various symptoms most of the time, which they either
self-treat or which resolve spontaneously without medical consultation.
Only a small proportion of patients who eventually consult their doctors
are referred to hospital for investigation [31–33]. This observation is also
true for rectal bleeding [34–36].

surprising, therefore, that advice on the management of rectal bleeding
stresses the importance of its detection [17,18] and prompt investigation
[4,5,19].
This advice has been supported by reports that rectal bleeding has a high
predictive value for cancer in primary care [4,5,19], and that it is impossible
to differentiate between rectal bleeding from benign and malignant
disease [4,5,19,20–24]. It is further supported by the unproven assumption
that early referral of all patients with rectal bleeding will improve the survival
of those with cancer [25].
These ideas have formed the basis for the current paradigm governing
the approach to the management of rectal bleeding, which advises an
aggressive policy of full colonic examination in all patients over the age
of 40 [4,5,19]. This is partly the reason for the serious mismatch between
demand and the resources for investigation—some cancer patients have
long waits to be seen due to the unnecessary investigation of patients at
very low risk of cancer.
We question these assumptions, and propose that it is possible to classify
patients on the basis of their cancer risk for different investigation
strategies.
The British government has recently introduced the “Two-Week Standard”
[26], which promises that all patients suspected by their GPs of having
bowel cancer will be seen within two weeks. This has focused attention on
the problem, which may be partly addressed by a reconfiguration of referrals
by identifying precisely which patients should qualify for urgent referral
and investigation [27–30]. However, it is yet to be seen whether this will
cause a greater delay in patients who do not fulfil these criteria, which in
turn might exacerbate the problem with no overall benefit to all cancer
patients.The introduction of referral guidelines [27–29] must not deflect the
government from the long-term solution, which is for a substantial increase
in hospital resources for all patients requesting and needing investigation,
not just those patients at higher risk.

There are currently insufficient resources to fully investigate all patients
with rectal bleeding to exclude the small possibility of cancer, and this is
the dominant factor in developing strategies for the management of rectal
bleeding. However, even if there were unlimited resources it may not be
desirable to investigate all patients because the small risks associated with
the investigative procedure might outweigh the benefits, particularly in
groups at very low risk of having cancer.
The importance of efficient as well as effective delivery of healthcare was
the subject of the Rock Carling Lectures delivered by Archie Cochrane in
1972 [1], and continues to be an essential aspect of clinical medicine. In the
context of the management of rectal bleeding, effectiveness is achieved if
all patients with colorectal cancer are promptly diagnosed, and efficiency is
achieved by limiting the number of patients without cancer investigated. In
view of the high prevalence of rectal bleeding in the community and the
potential demand for its investigation, the efficient management of all
patients presenting with rectal bleeding will profoundly affect the prompt
diagnosis of those with cancer.
The management of rectal bleeding as a symptom of bowel cancer begins
with advice to the general public through disease awareness campaigns,
proceeds through referral guidelines to general practitioners (GPs), and
finishes with the efficient use of resources for its investigation. The varying
prevalence and predictive value of rectal bleeding for cancer in different
cohorts of patients is important to all stages of its management.
Rectal bleeding is also important in the diagnosis of adenomatous polyps
[2–7] and colitis [7] as well as colorectal cancer [2,4–11]. Overall, 40% of all
colorectal cancers and 70%–80% of rectal and sigmoid cancers present with
overt rectal bleeding [9–11]. It may be a sign of an early-stage curable
cancer [12–15] and of large adenomatous polyps, which, with subsequent
colonoscopic surveillance, may be a valuable way of reducing the prevalence
and overall mortality from colorectal cancer [16]. It is perhaps not
